Prefix every chunk with its document title and heading path
A chunk that says "rotate the key, then restart the workers" is ambiguous. "Payments runbook / Key rotation / rotate the key, then restart the workers" matches the query on the words that matter. Split documents on their structure instead of a fixed token count, and keep the heading path on each chunk.
